JA Fellows Online Parent/Guardian Consent Form
Your student is taking a step toward his or her future by applying to be a
JA Fellow. This form should be reviewed & digitally signed by you and your
student to demonstrate your understanding of the commitment to be a JA Fellow.
In addition to reviewing the website & the
F.A.Q, please take the time
to review the information below.

Students must regularly attend all scheduled sessions, including announced
Saturday and special events, barring unforeseen circumstances,
and must notify their JA Fellows colleagues, JA Fellows staff & volunteers
in advance of any planned absences or tardiness. Students also understand that
should their attendance falter or should their behavior not reflect the high standards
of the JA Fellows program, their position in the program will be forfeit.
Students and parents/guardians also understand that action during the week
between sessions will be necessary for students to achieve success.

Communication between JA staff, volunteers, and students is vital. Therefore,
students must have an e-mail account that they check regularly. We also
encourage Parents/Guardians to provide us with an e-mail address so that we
can keep you informed about your student's progress.

Valid transportation must be secured for weekly meetings if MARTA or other
public transportation is not accessible.

Professionalism in demeanor and commitment is expected by students accepted
into the program.

The Student Application Process will consist of:
- Online Application (including essay)
- Online Recommendation (submitted by the Recommender)
- Online Parent/Guardian Consent Form (submitted by Parent)
- In-Person Interview
After students have completed the application and interview process and have
been accepted to the program, they are expected to follow all rules and
guidelines that have been explained throughout the application process. At any
time, if expectations are not met, students will and can be asked to leave the
program. We reserve the right to release students at will.

student & parent/guardian.
